- Old Passport
- Employment Contract (in English and Arabic)
- Intention to Transfer form (if the employee is transferring from one employer to another in Bahrain)
- Electricity Bill
- Employee address (if available)
- Medical Certificate (GAMCA Approved medical Certificate if out of Bahrain)
